Rail health assessments are a vital part of ensuring the safety and fitness of employees involved in rail operations. The Part A form is the initial step in the assessment process. Here’s a comprehensive guide to completing this form within the Carelever system.
Step 1: Access the Part A Form
Open the Part A form from the candidate’s or employee’s Carelever file.
If the rail health assessment is part of a pre-employment process, access their file in the Carelever system.
If the assessment is part of periodic health monitoring for an existing employee, access their file through Carelever Monitor.
Navigate to the Forms tab to access the online form.
Step 2: Verify Worker Details
The worker’s details will be automatically populated based on the information stored in their Carelever file. Review the information for accuracy.
Step 3: Complete the Rail Transport Operator Section
Name of Rail Transport Operator: This will automatically display your company’s name.
Supervisor/Contact: Provide the details of the contact person the Authorised Health Professional (AHP) should communicate with for any concerns.
Include their name, phone number, and optionally, their email or fax.
Date of Request: Enter the date you initiated the request. If the process began earlier, select the appropriate prior date.
Step 4: Fill Out Employee Details
Employee Number (if applicable): Enter the employee’s ID number if available. This field is optional.
Step 5: Specify Assessment Requirements
Risk Category/Level of Assessment: This field is automatically determined by the specific rail category medical you have requested. Do not alter this.
Job Duties Description: Indicate if your organisation has already provided KINNECT with a detailed description of the employee’s duties. If not, provide a short summary in the text box.
Type of Assessment Required:
Pre-selected based on the request type. Adjust only if necessary.
Options include:
Periodic Health Assessment: For existing rail workers due for their routine assessment.
Triggered Health Assessment: For workers requiring an assessment due to medical disclosure, new medications, or supervisor feedback regarding fitness for work.
Step 6: Detail Task-Specific Requirements (for Category 1 or 2 Assessments)
Colour Vision: Specify requirements based on the below criteria
Hearing: Indicate hearing requirements as per the below criteria:
Drug Screen:
Indicate if a drug test is required or not.
Musculoskeletal Requirements:
Specify physical tasks such as standing, sitting, lifting, and aerobic demands, including task frequency (e.g., standing for up to 6 hours per day).
Specific Tests:
Tick relevant tests required, such as cholesterol levels, HbA1c, resting ECG, or audiometry, depending on the category.
Step 7: Provide Supporting Information
Recent Health Assessments: Upload previous assessments if available.
Aids Required: Indicate if the employee uses aids such as corrective lenses or hearing aids.
Job Modifications: Detail any current job modifications in place.
Sick Leave Records: Submit significant sick leave details from the past 12 months.
Workers Compensation History: Provide details of any past or ongoing claims.
Critical Incidents: Document any critical incident involvement.
Workplace Reports: Share reports related to health or fitness for work.
Rail Safety Incidents: Include records of serious rail safety incidents.
