Pre-Employment Medical Assessment

Section 1: Core Components

  • Privacy and Confidentiality Disclaimer: Ensures the applicant's personal information is protected.

  • Explanation of Assessment: Details what the assessment involves and its purpose.

  • Declaration and Medical Release Authority: The applicant consents to undergo the assessment and authorises the release of medical information.
